Carlos Alcaraz clinched his sixth trophy of the season at the Cincinnati Open after Jannik Sinner was forced to retire just 23 minutes into their highly anticipated final.
Defending champion Sinner, who looked unwell from the get-go, trailed 0-5 in the opening set before calling for medical time out. Despite briefly attempting to continue, he was unable to carry on. The retirement handed Alcaraz his 22nd career title and a valuable boost in the race for Year-End No. 1. The Spaniard now leads Sinner by 1,890 points in the PIF ATP Live Race To Turin.
I’m so sorry for Jannik! Nobody likes to win because their opponent retires, especially in a final like this. Wishing you a speedy recovery! ❤️ Very happy with my week in Cincinnati and feeling ready for the US Open! 💪🏻🏆

“I’m super, super sorry to disappoint you,” Sinner told the crowd. “From yesterday I didn’t feel great. I thought that I would improve during the night, but it came up worse. I tried to come out, tried to make it at least a small match, but I couldn’t handle more, so I’m very sorry.”
The loss also snapped Sinner’s remarkable 26-match hard-court winning streak and raised doubts over his US Open title defense later this month. His scheduled appearance in the mixed doubles draw alongside Katerina Siniakova is also uncertain and almost certain to be withdrawn.
Heartbreaking.
And a great touch from Alcaraz. pic.twitter.com/drLptvQyHB

For Alcaraz, the victory further underlines his dominance in 2025. With a Tour-leading 54 wins and six titles this season, he heads to Flushing Meadows with a narrow advantage over Sinner in the Live Rankings. As Sinner defends last year’s US Open crown, Alcaraz will begin the fortnight with a 50-point edge in the standings.
